The programme

Structured Support Programme

A joined-up plan for people who would like longer-term support: from understanding where you are now, through building confidence and skills, to a gradual transition into work that respects your health.

Who it’s for

For people ready for joined-up, longer-term support

  • Adults recovering from long-term illness or mental health challenges
  • People who would like help in several areas — wellbeing, confidence, skills and work
  • People who want work to be part of their future, at a pace their health allows

How it works

The programme combines personal support, group activities and practical skills development into one plan. Instead of joining separate services one at a time, you have a single roadmap and a consistent point of contact.

Not ready for a programme? You can use any individual service on its own.

Programme stages

Three stages, one roadmap

The stages build on each other, but real life is rarely linear — you can move back and forth as you need.
  1. Stage 1: Assessment & planning

A supportive conversation-based assessment of your strengths, challenges and goals, turned into a clear roadmap you agree with.

    Support may include

    • Understanding your health, energy and circumstances
    • Identifying strengths and transferable skills
    • Agreeing priorities and a first step
  2. Stage 2: Support & development

Ongoing guidance, group sessions and practical tools to build confidence, manage day-to-day challenges and stay connected.

    Support may include

    • Peer support groups and wellbeing workshops
    • One-to-one mentorship
    • Skills training and volunteering
  3. Stage 3: Transition to employment

Focused preparation for work — confidence, skills and a gradual pathway into employment that respects your health needs.

    Support may include

    • Job search and application support
    • Preparing for employer conversations and adjustments
    • Planning a phased or gradual start

How the pace is adjusted

  • Your plan sets the speed

Each stage lasts as long as you need. Moving on is agreed together, never automatic.

  • Setbacks are expected

If your health changes, the plan changes. You can pause or step back a stage without starting again.

  • Regular reviews

You and your KND contact look at what is working and adjust the mix of support.
